DIETITIAN SERVICE

The Community Nutrition and Dietetic service (CNDS) is open to adults and children of all ages.

A referral from your GP or other healthcare professional (HCP) is required to see the dietitian in Primary Care.

Dietitians see people living with a broad range of medical conditions in which food and diet can help support and improve health.

These include heart disease, coeliac disease, irritable bowel syndrome, malnutrition, childhood obesity, faltering growth and diagnosed food allergy in children, in addition to many other conditions. Both phone and in-person appointments are available.

People living with type 2 diabetes, pre-diabetes and obesity can now be referred to the dietitian in the Integrated Care Chronic disease management hub. There are one to one consultations for all conditions and group education for type 2 diabetes (DISCOVER Diabetes) available. Please refer to the tab – Community Specialist Teams for Chronic Disease Management.
